Evaluation of Health Transformation on the Implementation of E-RM (Electronic Medical Record) in Community Health Centers

Abstract
Health transformation is the strengthening of the health system by increasing access and quality of health services towards universal health coverage. According to data from the PKP (Health Center Performance Assessment) of Pati II Health Center, the interpretation value for registration services reached an index of 83.3%. The obstacles experienced were service officers who changed too often and required long training for each new officer. In addition, there was another obstacle that many patients who came did not bring complete identification. This of course would hinder the queue at the registration section. The purpose of this study was to evaluate health transformation in the implementation of E-RM (electronic medical records) in the work area of Pati II Health Center. This study used a qualitative research design conducted in June-July 2024 with 6 informants who were taken using a purposive sampling technique. The results of the study in the input, process and output components have been carried out in accordance with the guidelines of the Minister of Health Regulation No. 24 of 2022 concerning E-RM. However, there are inconsistencies in several points such as the absence of specific officer training, officers often change, incomplete facilities and infrastructure and only 1 registration counter is provided. Reports are only done systematically, while external party reports are only SKM. In conclusion, the implementation of E-RM at Pati II Health Center still needs to be improved in officer training, human resources, facilities and infrastructure, E-RM reports and evaluations in order to improve the quality of E-RM services.
