The Relationship Between Delivery with Premature Rupture of Membranes and The Success of Early Breastfeeding Initiation

Abstract
The incidence rate of Premature Rupture of Membranes (PROM) ranges between 3–18% in preterm cases and 8–10% in term pregnancies. One of the causes of maternal death during or immediately after childbirth is infection. Maternal infections can occur during the antenatal, intranatal, and postnatal periods (Alim, 2015). Infections during the intranatal period are 65% caused by PROM (Muliatul, 2018). Several factors influence the occurrence of Low Birth Weight (LBW), one of which is PROM. PROM occurs in both term and preterm pregnancies; in term cases, 90% occur within 24 hours, while in preterm cases it often lasts longer. In pregnancies of 28–30 weeks, 50% go into labor within 24 hours and 80% within one week. This aligns with Nugroho (2012), who stated that PROM affects the incidence of LBW in deliveries at 34–36 weeks of gestation. PROM is a direct complication in pregnancy that affects maternal health and fetal growth in the womb, thereby increasing the risk of LBW. PROM can also cause oligohydramnios, which may compress the umbilical cord, leading to fetal asphyxia and hypoxia, as well as reduced nutrient supply and impaired fetal growth. The type of delivery experienced plays an important role in supporting the success of Early Initiation of Breastfeeding (EIBF). This research is an analytical correlation study with a cross-sectional approach, conducted in April–May 2025. The study population consisted of 22 mothers giving birth with a PROM diagnosis. The sampling technique used was total sampling, with all 22 mothers included as the sample. Data analysis in this study was conducted using univariate and bivariate analysis with the Chi-Square test, processed using SPSS Version 22 (CI 95% and α = 0.05). The results of the bivariate analysis showed a significant relationship between delivery and the success of EIBF (p > 0.001). However, the research findings indicate that there was no significant relationship between PROM delivery and the success of EIBF.
